Information Needed For Completion of Contract:
Legal business name and tax ID #.
DBA (Doing business as) name if different from legal name.
Business address.
Business type (ex. Dental office, hair salon, etc.)
Business phone, fax, and e-mail.
Date that business began operation.
Estimated credit card dollars monthly and average ticket.
Owner(s) name(s) and percentage of ownership.
Owner(s) home address, SSN, date of birth, and driver's license number.
Home phone number(s).There are two basic pricing methods for credit cards.
The first method bundles all of the different types of credit cards and places them into four categories:
Debit cards swiped - Will be the lowest rate.
Credit cards swiped - Rate is slightly higher than debit cards.
Rewards cards swiped or debit/credit cards keyed in - Generally called mid-qualified rate and is higher than debit or credit cards swiped.
Rewards cards keyed, corporate cards, or transactions not batched on a regular basis - Highest rates, and generally get the non-qualified rate.

The second method is called Interchange+ pricing. Interchange is the fee charged directly from Mastercard, Visa, or Discover for the 250+ types of cards that are issued. With Interchange+ pricing, you pay the interchange with an additional fee to the processor that encompasses our cost of doing business.
